My Bio: Victoria Pocos
Biography

I am a P4 student at the University of Findlay. I am currently completing APPEs, and my hub site is UH St. John Medical Center in Westlake, Ohio. I have previous retail experience at Rite Aid for about 2 years before the nationwide closure.

After I graduate in May 2026, I am looking forward to completing a residency and starting a career as a clinical pharmacist in an inpatient internal medicine or ambulatory care setting.

Ambulatory Care

I completed my ambulatory care rotation during the month of March. While at UH Meds, I worked with many different pharmacists. I was able to conduct primary care visits, complete culture call back calls, work with patients to do CMRs and submit MTM claims, and coordinate with the population health pharmacy team.

Cleveland VA Louis Stokes Medical Center
Inpatient Psychiatry Pharmacy
Preceptor: Megan Foreman

This month I was on the inpatient psychiatry unit. During this rotation, I was able to attend team rounds daily, complete discharge counseling, and lead patient groups about medication classes. I also monitored narrow therapeutic index window medications, such as lithium and valproate, and completed notes in the patient's chart.

Emergency Medicine Pharmacy
Preceptor: Maddie Volk

This month I was in the emergency department. I was able to respond to rapid responses, codes, and brain attacks. I dosed vancomycin orders, as well as adjusted patient antimicrobial therapy through the culture call-back program. I had lots of interaction with providers, and was able to present recommendations to them.

Community Pharmacy

This month I was in the retail setting through St. John's Meds to Beds program. This is a not a patient facing pharmacy, but dispenses discharge medications to patients going home from the hospital, as well as employee medications. I was able to get experience in counseling patients on many different types of medications. I also spent time at UH Westlake Retail Pharmacy, which was more of a traditional retail pharmacy. There, I was able to verify and fill prescriptions.

Firelands Regional Medical Center
Elective: Palliative Care

This month, I was in a palliative care clinic with a nurse practitioner. We conducted patient care visits in the clinic and at Seidman Cancer Center to provide symptom management and pain control. I was able to recommend many medications and titrate therapies as needed.

Special Populations: Intensive Care Unit

This month I was able to attend rounds with the ICU team and make pharmacological recommendations. We managed anticoagulation therapy and optimized antimicrobial therapy. I was able to lead a journal club and participate in daily topic discussion with the healthcare team.

This month I was able to provide medication education to many patients up on the floor, work on a longitudinal project across all UH hospitals, and spend time with the Med Rec, Meds to Beds, and Seidman Oncology Center teams. I was also able to round with the Internal Medicine team, where I saw many different disease states and the management of them.

Fall 2024 PHAR591 IPPE
This semester I completed my IPPEs at Fisher Titus Medical Center in Norwalk, Ohio. My preceptor was Ian Miller. This was my first rotation in a hospital setting, and I learned a lot. We did a lot of work in the ER and did a lot with antibiotics.

Spring 2024 PHAR 492 IPPE
This semester I completed my IPPEs at the Cleveland Clinic Cancer Center in Sandusky, Ohio. My preceptor was Kasi McKitrick. This was my first ambulatory care rotation, and I loved seeing the variety pharmacists do in their day-to-day jobs. I was able to observe in the retail pharmacy and the infusion pharmacy, and verify orders.

I completed my IPPEs this semester at Scarbrough Medical Arts in Findlay, Ohio. My preceptor was Carrie Scarbrough-Sampson. During this rotation, I learned a lot about compounding and the different techniques used to make many different medications.

My spring set of IPPEs was completed at Community Markets Pharmacy in Oak Harbor, Ohio. My preceptor was Jeremy Best. During this rotation, I completed my checklist and learned the process of dispensing medications. I also learned more about insurance and how that comes into play while dispensing.
