In this video reflection, I present my Portfolio Project from MCCG240 – Evaluation and Management Services, where I created a comprehensive Health Record Audit Training Document. This project is aligned with the course outcome of evaluating and correcting E/M codes and the institutional outcome of demonstrating information literacy and communication. Through this work, I applied 2023 E/M coding guidelines to real-world scenarios, analyzed provider documentation, and created educational materials to help front office staff understand accurate coding practices. I enhanced my knowledge of medical decision making and compliance, developed critical skills in documentation abstraction and training delivery, and strengthened professional behaviors such as attention to detail and clear communication. This project helped prepare me for the CCS-P exam and future coding roles by reinforcing my ability to apply technical knowledge in practical, real-world situations.
