Health Record Audit Training Document: Applying E/M Coding Knowledge

In this video reflection, I present my Portfolio Project from MCCG240 – Evaluation and Management Services, where I created a comprehensive Health Record Audit Training Document. This project is aligned with the course outcome of evaluating and correcting E/M codes and the institutional outcome of demonstrating information literacy and communication. Through this work, I applied 2023 E/M coding guidelines to real-world scenarios, analyzed provider documentation, and created educational materials to help front office staff understand accurate coding practices. I enhanced my knowledge of medical decision making and compliance, developed critical skills in documentation abstraction and training delivery, and strengthened professional behaviors such as attention to detail and clear communication.
