| Describe a scenario where you contributed to the assessment and diagnosis of a patient's condition based on symptoms, objective/diagnostic findings, medication history, and/or other clinical information. What were the differential diagnoses you considered (if any)? What diagnosis did you arrive at? Please specify if additional triage or diagnostic testing was required.: | During my rotation at Lowell Community Health Center with Dr. Azam Mariam one of the clinical pharmacist , I participated in a patient assessment during a mid-medication reconciliation for a patient with poorly controlled diabetes. While reviewing the patient's medication history and glucose logs, I noticed persistently elevated fasting blood glucose levels despite adherence to metformin and Jardiance. I discussed with the preceptor possible causes, including suboptimal dosing, medication errors, dietary inconsistencies, and potential insulin resistance. The differential diagnoses considered were type 2 diabetes with inadequate glycemic control, secondary diabetes due to medication interactions, or progression of the disease requiring therapy adjustment. Upon review, we discovered that the patient was taking Jardiance 25 mg three times daily instead of the prescribed once daily, which likely contributed to inconsistent glycemic control and potential adverse effects. After correcting the dosing and providing patient education, the preceptor and I monitored the patient's response over follow-up visits. No additional diagnostic testing was required at that time, but continued monitoring of A1c and glucose levels was planned to ensure efficacy and safety. |

If glycemic control remained suboptimal after correcting the dosing, I could have initiated or titrated adjunct therapy, such as adding a GLP-1 receptor agonist, considering cardiovascular and renal risk factors, or adjusting metformin dosing as appropriate. I would also provide patient education on medication adherence, diet, and lifestyle modifications to support therapy effectiveness. | Reflect on your APPE rotation experiences over the last 6 weeks as they relate to your career goals. Consider how your career goals changed or were reaffirmed after this experience. In 150 - 200 words, describe at least two events or experiences which reaffirmed your previous goals or made you reconsider them and state why.: | During my six-week APPE rotation at Lowell Community Health Center with Dr. Amy Sullivan and her team, several experiences reaffirmed my goal of pursuing a career in ambulatory care pharmacy, while also broadening my perspective on patient advocacy. One experience that strongly reinforced my career goals was counseling patients on diabetes medications and conducting medication reconciliations. For example, I identified a patient taking Jardiance 25 mg three times daily instead of once daily and was able to educate her on proper usage. This experience emphasized the critical role pharmacists play in ensuring medication safety, improving adherence, and directly impacting patient health outcomes. Another experience that shaped my perspective was assisting patients with insurance questions and navigating the 340B program. Many patients were unable to afford their medications without these resources, and helping them access care highlighted the pharmacist's role in patient advocacy and healthcare equity. Additionally, participating in vaccine clinics for RSV and Shingrix reinforced my interest in providing direct patient care services and preventive health interventions. Overall, these experiences reaffirmed my career goal of working in an ambulatory care setting where I can combine clinical knowledge, patient education, and advocacy to make a meaningful difference in patients' lives. |

I worked closely with the pharmacist, who used her expertise in patient counseling, while I handled documentation and ensured each patient was properly registered.
