IPPE Community Rotation Reflection Part 1

Write Reflection Here:: Leah Smith 10 October 2021 IPPE Community Reflection 1 During the first 2 weeks of my community rotation I was working at a company that I have been employed by for the last 5 years, CVS Pharmacy. Because I am already fully trained and experienced with the computer system and the way things generally run I was just responsible for vaccinations and then helping the pharmacy team during any down time. Vaccinating all day everyday may seem like the short end of the stick job to some, but for me I very much enjoy vaccinating and interacting with the patients one on one like that. However, I did feel that the staff and my preceptor included were at times short with me and sometimes rude. When I got upset and at my wits end with the way I was being treated and expressed my concern I was verbally abused by my preceptor. I was threatened by him that I'd fail my rotation and that he'd be calling the school and I was told I am not equal to him or his employees. Because of this, this experience could not have been any more negative. While I am not an employee, I am still a student intern working closely with the team helping them and trying to learn from them. That can't happen if I'm feeling like I'm being left out or having people be deliberately rude to me. No student should ever have to feel that way or be spoken to the way I was. This is supposed to be a pleasant learning experience and a hands on experience to see how a pharmacy functions. As someone who has worked in pharmacy for years, I know this is not an accurate representation so I really hate for someone with no experience be sent to this same place and have the same negative experience as me and not know that this isn't what it should be. I plan to make a formal complaint in hopes that no other student be verbally abused and degraded the way I was. I also am now thinking about becoming a preceptor myself at some point in my career to spread knowledge and positive experiences for those interested in pharmacy.

IPPE Community Rotation Reflection Part 2

Write Reflection Here:: My reflection will be based on the last 2 weeks of my rotation that took place at Price Chopper pharmacy store 179. My preceptor's name is Han Le, however it was not updated on the core elms to where I submit this. My overall experience at Price Chopper pharmacy went really well. I was able to learn how another retail chain pharmacy is run and learn their system. My preceptor was also really great at testing my knowledge and really putting my skills to the test by assigning small projects for me to research and talk about with her. Before coming to this pharmacy I had my mind made up that I disliked retail pharmacy and would prefer a clinical setting such as a hospital. After being at this site I gained a new positive perspective into retail pharmacy and I think I would maybe consider it as a possibility at some point in my career. I'm extremely grateful to my preceptor, Han Le, for teaching me new things and showing me that not all retail pharmacy is as stressful or wild as it seems.

IPPE Institutional Rotation Reflection Part 1

Write Reflection Here:: My institutional IPPE rotation site is at the Braintree Rehabilitation Hospital. Every morning of my rotation I'm expected to look through the new patient admissions from the day prior, gather a basic understanding of their diagnosis, review their active medication orders, and then go up to their room and review that information with them. This is so they have a basic understanding of what medications they will be given and for what while they are staying here in this facility. If the patients are not alert and or oriented, then I have to call their medical proxy/power of attorney or emergency contact and review their medications with them. This has allowed me to learn a lot that I never knew before about certain disease states. Just for example this past week I learned what Fournier's gangrene is as well as what a femoral endarterectomy is. Fournier gangrene is an acute necrotic infection of the genitals characterized by pain, redness, and sloughing of tissue. A femoral endarterectomy is the removal of plaque from the femoral artery. I've also learned about the flame trial; certain SSRIs such as fluoxetine being used to enhance motor function after an acute ischemic stroke. Many of the patients here are recovering from stroke and are sometimes put on fluoxetine usually 5 days after the event to enhance motor function recovery. This has made me feel extremely grateful and excited to learn new things that are of interest to me and has been an overall positive experience. I can't wait to be done with school and have a career working one on one with patients in a similar setting such as this one. Hopefully I can secure a job within the ambulatory care field by working hard and finishing school.

Write Reflection Here:: Having experienced working in an inpatient pharmacy was new and exciting to me as I've only ever worked in outpatient/retail pharmacies. I've been saying for some time that after school I want to work in ambulatory care seeing patients one on one during scheduled office visits and this rotation was able to give me a feel for that. Although it is not the same, I'm still seeing patients every day and working with them one on one. I did enjoy my time here and I'm grateful I was able to learn as much new information as I did and gain the experience of working directly with patients and charting information. This experience has made me believe that I would be a good fit for a career in ambulatory care. Working with patients on a day to day basis as well as charting and consulting with prescribers reaffirmed my original career goals of ambulatory care.

Please provide one example of an interprofessional observation and/or interaction that you had during this IPPE rotation in one to two sentences.: When going over the patient's medications with them they will often express the need for a medication . For example, if a patient is requesting their PRN tylenol I would easily tell the nurse the patient would like some tylenol can you please give it to them when you have a chance (assuming they did not have any recently). Also, when a patient has a maintenance medication missing from their med orders I would inform the doctor and suggest they prescribe it. For example, a patient was missing their short-acting insulin from their med list so I informed the doctor who then wrote an order for it (it was missing from the med list because the acute care hospital they came from accidentally left it off the discharge med list).

IPPE Institutional Rotation Reflection Part 2

Having experienced working in an inpatient pharmacy was new and exciting to me as I've only ever worked in outpatient/retail pharmacies.
