During rounds, I witnessed how different healthcare professionals came together to discuss patient issues and collaboratively create care plans. While I didn't have a formal role, I adapted by asking questions, learning from their interactions, and offering input when appropriate. Based on the values and skills you selected above, describe (in at least 5 sentences) how this experience made you think and act as future pharmacist.: | One of my interactions with diverse patient populations involved collaborating with a dietitian who requested a change to a patient's insulin regimen. The patient's feeding method had shifted from continuous feeding to bolus tube feeding. To ensure appropriate insulin management and prevent fluctuations in blood glucose levels, we recommended that the provider modify the patient's insulin schedule. Initially, the patient was on a Q6 sliding scale, but given the change to bolus tube feeding, we advised switching to a pre-meal insulin administration. This adjustment would allow insulin to be checked and administered before each bolus feeding, reducing the risk of hyperglycemia or hypoglycemia due to the feeding change. |

1.The entrepreneurship project helped me shift from focusing only on daily tasks to thinking more broadly about patient needs and how to address them efficiently. During my community IPPE, I thought more about targeting underserved populations, like youth, with initiatives such as smoking cessation. In my institutional IPPE, I saw how important sustainable planning and patient-centered services are, especially when it comes to counseling and medication access. The project gave me a better understanding of the business side of pharmacy and how much strategic effort goes into delivering high-quality care.
2. During my community IPPE, I saw how impactful medication therapy management (MTM) can be in improving safety and outcomes. In the hospital, I observed antimicrobial stewardship, where pharmacists adjusted therapy based on lab results highlighting their role in preventing resistance. We are doing well with pharmacist-led interventions and collaboration, but we could improve follow-up efforts and engagement for long-term care. Better systems for continued patient support would make a big difference.
3.In both settings, the primary customers were older adults, reflecting the demographics of the town I was in. In the community, patients needed help managing chronic conditions through counseling, medication organization, and adherence support. In the hospital, they faced more acute needs like managing complex medication regimens and transitions of care. I noticed a gap in consistent follow-up and patient engagement, especially after discharge or during long-term treatment.
4.One solution could be implementing Telehealth behavioral support, especially for rural or underserved populations, to improve access to ongoing counseling. Other ideas include using mobile health apps to support medication adherence and QR codes on educational materials to connect patients with helpful resources instantly. These tools could help sustain behavior change without relying solely on more staffing. They also meet patients where they are, using technology to keep them engaged.
5. In community pharmacy, steps like defining the program, market research, and developing a clear marketing strategy especially through schools and social media can improve outreach and patient satisfaction. In institutional settings, focusing on clinical requirements, defining operations, and setting a clear action plan (like assigning staff roles and tracking progress) ensures safe and efficient medication delivery. Both settings benefit from a structured plan that aligns services with patient needs while staying sustainable.
