A seeker of ways to make a difference while supporting the success of others.
Career
My career aspiration is to serve as a leader by inspiring others to be innovative and strive for success in their journeys for career and life success. I will obtain the required certifications needed to fulfill job requirements for leadership positions within my field. To achieve this, I will speak with my leaders about their career development and explore qualifications for advanced positions to determine which credentials are needed.
Personal
My personal ambitions are to visit and enjoy the many natural marvels in the United States, share memorable moments with my loving children and grandchildren, be kind to others and myself, and encourage others to achieve great things.
The privacy and security issues involved with EHRs and the release of information and the concepts of managing data and EHRs are the core of this project. Each week we created, retrieved, and analyzed patient EHRs. This project is a reflection on the guidelines and rules for the release of patient information (ROI), Master Patient Index’s (MPIs), and Health Insurance Portability and Accountability Act (HIPAA). It is a rumination of the responsibilities of maintaining accurate patient records, adhering to laws, and practicing in a way that protects your patient’s well-being. Because the project was created in an academic environment in regards to professional guidelines, rules, and processes, and encompassed collaboration with both our teammates and instructors, it is an ideal example of an outcome resulting from my individual growth by means of social, academic, and professional participation.
In order to grow in my future endeavors, it is vital that I master the use of all avenues for grownth Medical Chart Contents and Storage for Electronic and Paper Charts (.docx) 0.04mb
Each week we would focus on a different body system and diseases associated with the system. The weekly assessments were a portion of a cumulative analysis of a patient’s complaints and possible conditions and treatments. This included our understanding of laboratory results. based on lab work results. Each week we would explore the case from a different perspective by satisfying presented questions. The lab work results, as did other things, presented us with a need for mathematical reasoning and addressing our weekly questions required we come to conclusions and make treatment decisions. In the field of medical coding, we are required to understand patient charts. Thus, we will be faced with many medical scenarios that we must analyze and interpret with accuracy. Our conclusions and actions are direct reflection of skills in established methods of inquiry and our mathematical reasoning. 220928054120_HCAS_143_Portfolio_Project_Culminating_Case_Study.docx (.docx) 0.05mb
Each week we were presented with different conditions. The scenarios and tasks varied each week to maximize our experience from many viewpoints. This project is an example of one week’s assignment with a focus on injuries suffered from a fall off a rock wall. Our goal was to strategically analyze the interrelationships within and between anatomical and physiological systems of the human body and communicate our analysis in a written essay. This form of study and communication directly mirrors analysis techniques vital to coding. It lead to the routine I used today when tasked with coding patient cases. I first analyze the case from different perspectives to ensure I clearly understand the condition and the treatments. With that confidence I am able to effectively and accurately speak to the case if needed and accurately code the case. Research and Writing for the Workplace
Throughout the course I constructed a workplace document and engaged in the introduced writing process by taking my instructor’s feedback and employing it to improve the assignment. The linked document is a reflection on my experience. I speak to my discovery of my existing writing process at that time, what worked well, and how I will improve based on what I have learned throughout the course. A career in medical coding requires collaboration both in person using current technology. The nature of the job requires effective and efficient visual, written, and oral communication. The project is a direct reflection of quality communication. Informal Career Plan (.docx) 0.04mbResearch and Writing for the Workplace

It is a rumination of the collective responsibilities of maintaining accurate patient records, adhering to laws, and practicing in a way that protects a patient’s well-being. By following the guidelines, policies, external regulations, and standards as provided, we collaboratively ensure the accuracy and completeness of a patient records. Achieving this goal ensures the safety of our patients and ensures high quality medical experiences. Medical Chart Contents and Storage for Electronic and Paper Charts (.docx) 0.04mb
This project demonstrates the three critical components of a compliant Clinical Documentation Improvement (CDI) program in the form of a training guide. The three critical components of a compliant CDI program include: documented mandatory physician and coder education, detailed compliant query documentation, and CDI policies and procedures. The training is to be used in staff training to address a decrease in the case mix index (CMI) which suggests poor documentation or inaccurate coding. The training outlines the general process in preparation for an audit of clinical documentation. The proceeding outcome will ensure the accuracy and completeness of patient records as defined by policy, external regulations, and standards.

In addition to providing the diagnosis codes, rational and applicable guidelines used behind code selections, including format, symbols, and instructional notations, it demonstrates my knowledge of ICD-10 CM diagnostic coding in the form of an education training plan, outlining the primary goal of the training program, and a timeline of the training event(s). By demonstrating my knowledge and skill in the field of CM coding I can project my competencies for the field. Additionally, I am able to demonstrate my skills in abstracting information and applying my leadership skills that are needed in the collaborative world of medical coding. In this assignment I created an in-service Health Record Audit Training document to be used when teaching and training E/M coding to front office staff. To fulfill my job responsibilities, I must demonstrate my ability to meet all required attributes of an effective and efficient medical coder and leader. To demonstrate the needed competencies, I have attached a project that includes all pertinent information needed to successfully understand and apply E/M coding. It begins by defining E/M coding. From there it covers CPT code symbols, punctuation, abbreviations and everything else up to and including how to apply what they learn to two coding scenarios. 220928010106_MCCG_240_Portfolio_Project_How_to_Assign_Evaluation_and_Management_Codes.docx (.docx) 0.05mb
