Please provide one example of an interprofessional observation and/or interaction that you had during this IPPE rotation in one to two sentences.: Patient is taking Eliquis at home which is an anticoagulant. The physician would like to administer a dose of Heparin 5,000 units SQ. This is used as a prophylaxis. It helps prevent problems that could arise from the patient being dormant and not moving while in the hospital. Since the patient is taking Eliquis at home there is no needed for the Heparin. Pharmacist reached out the physician about the duplication. The intervention was approved.

Write Reflection Here:: Something that has been so surprising is the number of medications on back order. Patients are eager to receive their medication in a timely manner and may not understand when and why a medication is on backorder. But I see the effort from the pharmacist and the technicians to assure the patient they will get their medications as well as ordering the medication to ensure the patient can receive their medication as soon as possible. Their efforts take this negative situation and give it a positive outlook. I feel this way because you can see the dedication from the pharmacy staff and empathy for the patient. This situation that happens to occur in the pharmacy daily shows the empathy from the pharmacist to the patient. Something I plan to take away from this rotation and something I have learned is the frame of mind of the pharmacist. Seeing how compassionate, empathetic, and positive the pharmacist is, is how I plan to be with future patients. Since my rotation site is in the cancer portion of the hospital, I hope to leave a positive impact the way the pharmacist leaves on his patients.

Write Reflection Here:: I enjoyed being an intern at Valley Health Pharmacy retail pharmacy. I have only worked in retail for a few months prior to the COVID-19 pandemic. I worked at a retail chain pharmacy and didn't really enjoy the environment. The environment was very fast paced and there wasn't much time in the day to have a one-to-one conversation with the patient. Even though I tremendously enjoyed this experience I believe that these four weeks have reaffirmed my future career goals. Two events that made me consider sticking to my initial career goal was that most of the day consists with dealing with insurance companies. I see the compassion and determination from every technician and pharmacist assuring that the patient is receiving their prescription and at a low cost. I find this to be very important but just not something that I would look forward to doing every day. Another aspect is dealing with manufacturer of medication. I personally feel that there is less doctor-to-patient interaction. Having one-on-one interaction with the patient about their medication, medication reconciliation, or simply just counseling the patient is an aspect of my day that I will look forward too. I believe that all these experiences have reaffirmed my career goals in wanting to pursue a career as a clinical pharmacist in a hospital setting.

IPPE Institutional Rotation Reflection Part 2

Write Reflection Here:: Before starting this rotation, I had my mind set on achieving a career in industry. Now that the four weeks have come to an end, I must admit this it has sparked an interest in working in a hospital setting. One instance that sparked my instance was when I spent a few days in the IV Room. In the class setting we learn about the techniques, garbing, and procedures that take place in the IV Room. Getting to garb, see the calculations, and the medication being prepared were very interesting. The day consisted of seeing the medication order, what the medication is used for, and verifying the dose. This may sound like the normal process that is done in the main pharmacy, but the environment was completely different. Another instance that sparked my interest in the hospital setting was working in the Emergency Department. Not only was the pharmacist so knowledgeable about every medication order and literature pertaining to that order. But the environment was very thrilling and exciting. Withing fifteen minutes of being in the ED there was a code called. Prior to being at this institution I was not aware that a pharmacist needs to be present when a code is called. The four weeks that I have been at Hackensack I have been able to see the true meaning of pharmacy being a broad career path.

Raising awareness about autism is essential to promoting understanding in society. It encourages diversity and gives people with autism the tools they need to succeed in a variety of settings. As frontline healthcare providers, pharmacists are essential to raising awareness of autism because they offer support and direction to people with the disorder and their families. They make ensuring that essential medications are accessible, give guidance on how to manage related health problems, and create an inclusive and welcoming atmosphere for people with autism. Pharmacists can greatly improve the general well-being and quality of life of people on the autistic spectrum by learning about their specific requirements. Utilizing education, empathy, and advocacy, pharmacists play a role in fostering a more inclusive culture that values and supports people with autism.

De-prescribing medication is crucial for maximizing health benefits and reducing potential adverse effects, particularly for the senior population. First of all, reducing the number of unnecessary medicines could decrease the chance of negative drug interactions and reactions, which are more common in older persons because of physiological changes brought on by aging. Second, by streamlining prescription regimens, de-prescribing encourages drug adherence and makes it simpler for senior patients to properly manage their prescriptions. Thirdly, it can lessen the financial strain that pharmaceutical expenses place on consumers and healthcare systems. Furthermore, by lowering medication-related adverse effects and enhancing mobility and cognitive function in senior citizens, de-prescribing can enhance overall quality of life. By doing comprehensive medication assessments, identifying possibly inappropriate prescriptions, and working with healthcare providers, pharmacists play a critical role in de-prescribing.

Co-Curricular Activity - Elective #2
This talk was based on the development of the new OTC contraceptive, Opill. The development of over-the-counter (OTC) contraception has significantly improved reproductive health by giving people more access to safe and reliable birth control methods without a prescription. In order to guarantee the appropriate use and accessibility of over-the-counter contraceptives, pharmacists are essential. They may provide helpful advice to people looking for these items, assisting them in navigating the variety of possibilities and choosing the best course of action according to their particular requirements. Additionally, pharmacists may offer vital advice on how to use contraceptives correctly, possible adverse effects, and efficacy, enabling people to make knowledgeable decisions about their reproductive health. To further ensure their patients' safety and wellbeing, pharmacists can provide screening services to find contraindications or possible hazards related to over-the-counter contraceptives.

Write Reflection Here:: The word that comes into mind when I think of this institutional rotation is Adventure. Everyday has been an adventure. Seeing where I'm stationed for the day, the tasks for the day, and so much more. During the semester when we were learning pharmaceutical calculations, I was constantly asking myself "Is this something I will really have to do while being a pharmacist? Do I really need to know how to do these calculations?" While shadowing one of the pharmacists I got my answer, which is yes, these calculations will benefit me in the future. The pharmacist was checking to verify one of the med orders when she noticed that physical requested the dose in drops. I was asked to help with the math. I felt really involved and felt prepared to answer the questions the pharmacist had. Whether it was how many drops in 1 milliliter or if I knew how to do the allegation method. After answering all the questions, I felt very proud of myself that I was able to implement all the information that I learned in the classroom to the institutional setting. From This experience I feel very confident that I am capable to answer the questions the pharmacist and that I can achieve anything I set my mind too. This rotation is an adventure that I look forward too every day.

Please provide one example of an interprofessional observation and/or interaction that you had during this IPPE rotation in one to two sentences.: Due to this pharmacy being located in the hospital we receive a handful of surgery orders. It's very impressive to see how closely and how well all of the physicians work with each other and making sure the patient is treated with the upmost care and receives all the medication needed for before the surgery as well as after the surgery. In the process of doing so we are not just consulting with the patient but also the physicians, nurses, and/or care takers. Luckily I got to help prepare and fill mostly all of the surgery orders.

Had the pleasure attending Dr. Bartlett's Book Launch, MEDSTRONG. I was very interested to hear about the book she was constantly mentioning in class. The book highlights the importance of de-prescribing and optimizing in older adults. Having Dr. Bartlett as a professor in the Healthcare course I was able to see the significance this could have on the overall health of a person. I believe it is necessary for patients to be in constant contact with their PCP's in order to ensure their medication is working as it should and that they see an improvement in their health. But what I learned from Healthcare and from this book launch is that Pharmacist play a huge role in this as well. It is essential to look over patients medications. To ensure it is being take correctly, refill on a schedule basis, and that the patient is starting to feel better. But is it also important to notice when a prescriber has been giving a certain medication for too long.

The guest speaker for this activity was Sarah Lynch, PharmD, and BCACP. She is a Clinical Associate Professor and has published literature in the area of student and pharmacist training in hormonal contraceptive prescribing. Growing up in a Middle Eastern home, contraceptives were never a topic of discussion. Even at 23 years old this is still not a topic that is formally discussing in a traditional Arab household. The foundation that I have in regards to Contraceptives has been given to me from my education. Even then it was never fully informative. I found this zoom meeting to be very informative overall and eye-opening in regards to contraceptives. In regards to professionalism, we discusses the standards for pharmacists when training/procedures for hormonal contraceptives. We also talked about certain challenges we might encounter in order to help provide better care. Overall, I believe that this zoom session was very informative. I believe that there should be more sessions on contraceptives.

Elective 1: Breast Cancer Walk (4.2 Leadership 4.1 Self-Awareness 4.4 Professionalism)
This 5 Mile walk was very moving and touching to me personally and as a student. First, I come from a family where cancer has impacted us tremendously, especially breast cancer. It was very inspiring to see all the survivors and family members of those affected in the walk and how passionate they were. In regards to student development, I was very much looking forward to the walk once I heard about it through Dr. Campbell. Even though the field of pharmacy gets very hectic and it is necessary to take time out of the work space to do something for a good cause. Every doctor that has left a positive impact on me is one that is able to take time out of their day to give attention towards. This walk has taught me an important lesson to take time out of the day, no matter hectic it maybe to do something that is benefitting and for a good cause. This is something I hope to keep no matter how challenging life may get.

  1. Description – What past experiences have influenced your career goals? 
    1. Some past experiences that have influenced my career goals have been the times that I have been sick. The past year I have found myself to be in and out of the emergency room due to gastrointestinal issues. Due to this, I have communicated with my pharmacist more than usual. And due to this I found the field of pharmacy to be so intriguing and much more than dispensing.
  1. Feelings – What are your feelings regarding your career goals (anxious, optimistic, etc)? 
    1. I am very anxious about the future. Especially this semester. As the days go by I feel the pressure to try to understand what I want to do in the future. But I am very optimistic that the decision I do end up making I will love. I am more anxious when it comes to thinking about the residence or a fellowship.
  1. Evaluation – What is your evaluation (positive or negative) of your experiences and why do you feel this way? 
    1. So far most of my experiences are positive. I still think about the APhA Career Pathways Activity from last semester and spend most of my time envisioning what I want to accomplish in the future. Overall, my evaluation is very much positive and optimistic. The past trained me to be trained for the present.
  1. Analysis – What did you learn from your experiences and how does it apply to your current career goals? Have your career goals change after your participation in the MCPHS Curriculum? 
    1. Recently, this semester we have talked multiple times about concentrations, residency, and fellowship. Which to me is what I intend to do. At first, when hearing about these topics, I thought to myself that I wouldn’t make it. But if the past has taught me anything is that I am capable. I know that I can accomplish anything I put my mind to. MCPHS is motivating me to continue with my future career goals
  1. Conclusion/action – What actions do you plan to take to achieve your career goals? Do you plan to pursue any post-graduation education or additional training? 
    1. I plan to continue doing well, like last semester. The past semester does not compare to the level of difficulty this semester, but I know I can accomplish anything I set my mind to. I plan to do well this semester, find out about residency programs, work on my CV, and even look at fellowships. This semester will consist of conducting research about things that will interest me and help me achieve my future goals.

IPC 1: Roles and Responsibilities
Coming into this informative session I was under the impression that I had known everything required for other healthcare professions through my friends but I was very intrigued with the requirements for dental hygenist. I didn't know that they only require an associates degree. I was aware of the responsibilties for pharmacists. I was very surprised to see all the responsibilities for the different healthcare professionals. I was intrgued especially by Physician Assistant. It was very interesting to see how much they are capable in treating their patients compared to a physician. I was also very surprised by acupunterist. I didn't know that an acupuncturist can have a work setting in a hospital. This was a very positive experience. My group and I were very interested in knowing what each individual can and cannot do in their healthcare profession and how we can all work together in the work setting. Personally, I believe two brains is better than one. When healthcare professionals work together in a team it's easier to receive successful outcome in the patients health outcome. Through this, each individual on this team will be able to provide insight on their specialty and will aid in providing the patient with overall better care. One step that I will take is knowing what everyone one my team is capable off. It's important to know each person's high point inorder to not over step.
