Write Reflection Here:: My time working at a non-profit community pharmacy deeply influenced my career goals. I saw the pharmacy team's strong commitment to providing excellent care and making sure every patient's well-being came first. This experience made me admire the pharmacy field even more, especially when it comes to helping people in tough situations like refugees and those with money struggles. One particular moment showed me how dedicated the pharmacy was caring for its patients. A patient urgently needed medicine but only had $15. The pharmacist went above and beyond, working hard to lower the cost of the medicine from $50 to just $10. This kind gesture ensured the patient could afford their medicine, and it encouraged them to stick with their medication adherence. Seeing how the pharmacy positively impacted the lives of people in need inspired me. It confirmed my desire to become a pharmacist who focuses on helping the community and providing patient-centered care. What impressed me the most was how well the pharmacy worked with other healthcare providers, like doctors. In urgent situations, such as when a patient needed medicine for surgery, the pharmacists and doctors worked together smoothly to make sure the patient got the right treatment on time. This teamwork ensured patients received personalized care, resolved medication issues quickly, and made sure everyone had a full picture of the patient's health. My future career goals are firmly based on this meaningful experience. I'm determined to follow the example of dedication and positive impact I saw in that non-profit community pharmacy. My dream is to become a pharmacist in a similar setting, where I can provide caring and effective healthcare that truly improves the lives of people who need it.

IPPE Community Rotation Reflection Part 1

Write Reflection Here:: IPPE Community Rotation Reflection Part 1 I. Description - Describe the event. Two days ago, at my community rotation site, I had a patient come in the pharmacy requesting counseling on her symptoms. My pharmacist had told me to shadow him on his counseling procedure with the patient and to try to see with my academic expertise of counseling what I would say is appropriate for the symptoms. The patient had first expressed that she had a bad cough that she has been experiencing for a few days. With this information I thought cough medicine such as Deslym would be appropriate. However, the patient then added that it was so severe it was resulting in her coughing up blood and that she believed it may be due to tuberculosis. II. Feelings - Describe the feeling you experienced during this event. This interaction made the pharmacist and I both very perplexed. The fact that they believed their symptoms may be due to this severe disease state, yet they were here in the pharmacy not wearing a mask or warning others of her condition was a very uneasy feeling. All we could do was advise her to go see a doctor immediately and in the meantime try to avoid exposure and wear a mask. III. Evaluation - What is your evaluation (positive or negative) of your experience? It was a positive experience in the sense that I got to have an unusual interaction. IV. Analysis - Why do you feel this way? This interaction made me realize what being in the healthcare field is like and what similar situations I may encounter, and that made it positive in my learning experience. V. Conclusion/Action - What actions do you plan to take based on what you learned from this experience? I plan to be ready for any possible situation with my patients. Many people come from different backgrounds and other countries that carry many diseases and require aid. In some instances, we as pharmacists will be the ones to advise them on what to do.

Please provide one example of an interprofessional observation and/or interaction that you had during this IPPE rotation in one to two sentences.: During my first week, I observed a scenario involving a pharmacist and a physician who were collaborating on a patient experiencing a vancomycin infusion reaction, commonly known as red man syndrome. This adverse reaction is characterized by symptoms such as a red rash, hypotension, tachycardia, and angioedema. Complicating matters, the patient had an allergy to Benadryl, ruling out the use of diphenhydramine. Witnessing the pharmacist and physician working together to identify the most suitable medication for the patient's unique condition was a fascinating experience.

As this was my first time working in a clinical institutional setting, I was not sure of what to expect. At Umass Memorial, the inpatient pharmacy had a very different pace than what I had been used to in outpatient pharmacies. On a daily basis, I observed pharmacists as integral members of the healthcare team, working closely alongside physicians, nurses, and various other healthcare professionals. The pharmacists played a critical role in the management of medications for inpatients or residents of healthcare facilities. Their focus was on delivering comprehensive pharmaceutical care, encompassing tasks such as medication therapy management, dosage adjustments, vigilant monitoring for potential drug interactions, and ensuring the safe and effective utilization of medications for patients receiving care within hospitals or institutional settings. However, I couldn't help but notice that direct patient interactions were relatively scarce. The majority of recommendations and counseling occurred through interactions between pharmacists and physicians or nurses, with the information then being relayed to the patients.

As much as I appreciated the seamless and convenient connection that pharmacists maintained with physicians to ensure precise treatment, I couldn't help but feel a sense of disappointment regarding the limited direct interaction between patients and pharmacists. Personally, I place a high value on such interactions and envision myself as a pharmacist who thrives on building meaningful connections within my patients in the community. I aspire to engage with patients regarding their concerns and providing tailored guidance. Therefore, as a future pharmacist, I see my calling in the role of an outpatient pharmacist. This path would lead me to work in a community or retail setting, allowing me to serve a diverse array of patients with my primary focus being on the accurate dispensing of medications and the provision of valuable medication-related guidance.

IPPE Institutional Rotation Reflection Part 1

I. Description - Describe the event.

During my initial week of clinical rotations at Umass Memorial, I had the opportunity to participate in an "escape room" project orchestrated by the graduating nursing students at our institution. This unique activity revolved around an SBAR case involving a patient facing respiratory distress, coupled with additional risk factors such as obesity, hypertension, and recent breast augmentation surgery, which complicated the patient's situation.

In the escape room scenario, I collaborated with a pharmacy resident, pharmacist, registered nurses, and various other healthcare professionals, with the aim of deciphering the intricate puzzle presented by this patient's case and progressing through the stages. At each stage, we encountered additional health-related questions and obstacles that required resolution in order to devise the optimal solution for the patient's complex situation.

 II. Feelings - Describe the feeling you experienced during this event.

I found this event to be a truly fantastic experience. It provided an enjoyable avenue for active engagement in an educational context, fostering teamwork among healthcare professionals as we tackled the case together.

III. Evaluation - What is your evaluation (positive or negative) of your experience?

My evaluation of the experience was incredibly positive. The nurses, pharmacists, and other professionals with whom I collaborated were exceptionally hospitable. They went out of their way to involve me and encouraged me to share my insights on how we could successfully unravel the puzzle. They appreciated my perspective and thought process, even though I had limited knowledge in the field. This, in itself, boosted my confidence and motivated me to contribute actively to our collective effort in the escape room.

IV. Analysis - Why do you feel this way?

I feel this way because it was great to work in an Interprofessional collaboration. Although the SBAR case we tackled was a simulated scenario, it provided a profound glimpse into what real-world cooperation with healthcare professionals would entail. Our collective effort facilitated the smooth resolution of the puzzle, underscoring the potential to enhance patient outcomes in actual clinical settings through effective teamwork.

V. Conclusion/Action - What actions do you plan to take based on what you learned from this experience?

In my prospective career as a pharmacist, I intend to implement the skills I observed from this experience in my future interprofessional collaborations. These skills will be applied through effective communication, focusing on the well-being of the patient, and working on problem solving effectively with any health care professionals that are on my team trying to solve this puzzle.
