Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: I was able to work with a pharmacy resident in prescribing the emergency contraception Ella to a patient. The patient's BMI was higher than recommended for both Ella and Plan B, so we consulted with the physician for the patient to receive a copper IUD for emergency contraception that would be most effective with her BMI.

During the meeting, we discussed how to increase the vaccination rates among staff members and patients, along with ways to maintain safety parameters in the work place.

Write Reflection Here:: The event that I was the most engaged in was when a prescriber sent in an Augmentin prescription for a patient that had a penicillin allergy listed in their profile. The profile showed that the patient had picked up Augmentin before, but it was from a different prescriber so this could have been the instance that the patient had the allergic reaction. The preceptor asked me to call the doctor to find out if they knew about the patient's allergy. I called multiple times throughout the day, but the prescriber did not answer and the voicemail box was full. I left a message for the patient as well to contact us so we could confirm this allergy before filling the Augmentin. I could not get ahold of either, so finally I called an alternative phone number in the prescriber's profile at another office. They answered, but informed me that she was not there that day. I told them I could not get ahold of her and I needed to before this medication can be filled. They left a message with her on her direct phone and finally she called us and sent in a new prescription for cefdinir. I felt very frustrated during this because it didn't make since that the prescriber could send this over and then not be reachable for questions. After finally getting it solved for the patient, I felt relieved. This was a very positive experience for me, because it really showed how valuable pharmacists are. If it wasn't for us calling, the patient could have had another severe allergic reaction. I plan to always put the patients safety first and not fill something that could possibly harm them. If something doesn't seem right, I will make every effort to call the prescriber to make sure the patient is safe.

IPPE Institutional Rotation Reflection Part 1

Write Reflection Here:: During this rotation, a patient was prescribed diazepam rectal suppositories, and the insurance would only cover the generic. The generic was on a manufacturer backorder, so only the name brand Diastat rectal suppositories were in stock. The insurance would not cover the name brand ones that were in stock and they were very expensive. The pharmacist did not want the patient to go without this medication, as withdrawal seizures could occur. The pharmacist, with the help of technicians, got the insurance to do an override for this time so that the patient wouldn't go without their medication. During this, I felt very frustrated at first because it is disappointing that it is so possible for patients to not get the medications they need because of high prices. But, once the insurance applied the override, I felt very happy that it worked out. This was a positive experience for me because it showed that we should not give up because our patients rely on us for their health outcomes. Pharmacists help patients achieve better health outcomes and I am proud to be on the path for this career. When I am a pharmacist, and even now at my pharmacy intern job, I will continue to advocate for my patients and put in the extra work to help them get the medications they are prescribed in order to live healthier lives.

Write Reflection Here:: The last 4 weeks at the Norwich Walmart Pharmacy were very eye-opening. I have worked at CVS Pharmacy for 4 years, so it was extremely beneficial to be able to learn a new system and see how a different branch of retail pharmacy is run. An experience I had that reaffirmed by career goal of being a retail pharmacy manager was seeing how all the techs got along with each other and the manager. They were like a family, never called out, and always helped out each other to get tasks done. This made me feel hopeful in the area of community pharmacy since where I have worked there has always been call outs which makes it very hard on the other employees. Another experience I had that reaffirmed my goal was seeing how the manager was so kind and close with her patients. The patients would always come in looking for her and she had a great professional relationship with them. This reassured me that retail pharmacy is all worthwhile because we are truly impacting people's lives and helping them be healthier.

Write Reflection Here:: This rotation was a great experience for me because I was able to see a pharmacy environment other than retail. All I have had for prior experience in pharmacy was in the community setting, so that is what my career goals have always been. After working in a long term care pharmacy setting, I was able to see that there are many options out there for pharmacists to work in. Something that reaffirmed my career goal of community pharmacy was that in this institutional setting, there was not as much face to face conversation with patients as I have had in retail, which I really like about retail. Something that made me reconsider my previous goal of community pharmacy was that I really liked how in this institutional setting, there were different levels/types of technicians such as filling techs and data entry techs. I like how each person had a specific job to get work done, which made it a much calmer, more efficient workflow than in a retail setting. This was a great experience for me because it showed me that I enjoy working in multiple different pharmacy settings and can open up my options outside of retail pharmacy.
