Anticipated Graduation 2025
From time to time the physicians, residents pharmacists and I will deliberate on patients' specific cases and what treatment decisions would be appropriate for these patients. This is usually done before and during rounds. These deliberations gave more understandings and knowledge on why an intervention is made based on patient's specific needs.
My best work in this rotation was a chart I made for the technicians to make things easier for them. This chart contained all the medications in the hospital formulary: their beyond used dates, storage information, and specific instructions on how they are used. Below is an attachment of my best work.

What I enjoyed most in this rotation was engaging with patients to provide education and support for managing their conditions.
This rotation also helped strengthen my communication skills by working closely with an interdisciplinary team, including doctors, nurses and pharmacists.
My best work in this rotation was a handout I created to give to patients in a smoking cessation class we had at the clinic. Attached is a copy of the handout.

One of the most important lessons I learnt was importance of medication error prevention. This topic was very important to me that I had to do a presentation on it. Why doing my research on this topic, I came to understand the potential for human error, systemic failures and the important role good communication can play in medication safety and overall person safety.
My best work for this rotation was research I did on REMS program, and what the Adena Hospital can do to be in compliance of this program in accordance with FDA requirement. Attached is the report I presented to the Adena Health system.

The rotation has help me to improve on my ability to access medication regimens for patients with multiple comorbidities.
Another key takeaway was developing my communication skills with both healthcare providers and patients. I had the opportunity to provide medication counseling, clarify prescriptions with physicians, and make recommendations.
Overall, this rotation has strengthened my clinical skills and reinforced my passion for pharmacy.
My best work in this rotation was a journal club presentation I made comparing Vancomycin and novel glycopeptides. Attached below is my best work.

Rotation Type: Special Populations 2
Preceptor: David Carr
Site: Holzer Medical Center, Gallipolis.
Description: During my special population rotation, I had the opportunity to work with multidisciplinary team in a long-term care facility, where we cared for the elderly. This rotation was focused on the clinical management of patients with comorbidities, including diabetes, depression, dementia, and chronic pain. Some of my key learning experiences included:
- Counseled patients and caregivers on the proper use of medications and recognition of potential adverse effects.
- Performed medication review tailored to the unique pharmacokinetics and pharmacodynamics in the elderly patients.
- Collaborated to ensure compliance with CMS guidelines by focusing on reducing the inappropriate use of antipsychotics in dementia patients.
- Involved in interprofessional rounds with physicians, nurses, my preceptor, and social workers.

Advanced Pharmacy Practice Experience 4 (oct. 2024)- Ambulatory
Rotation Type: Ambulatory
Preceptor: Trevor Slone
Site: Holzer Medical Center, Jackson
Description: My ambulatory rotation helped me gain skills, knowledge and experiences essential for providing high-quality care in an outpatient setting. During this rotation, I was immersed in the outpatient care environment, managing patients who require preventive, diagnostic and chronic disease care. This rotation provided me with the unique opportunity to follow patients over multiple visits. This structure helped foster my understanding of managing long-term health and chronic conditions outside the hospital setting. Some of the experiences I gained in this rotation include,
- Mastering chronic disease management, focusing on diseases such as diabetes, hypertension, asthma, and COPD
- Engaging in preventive care practices, including INR checks to prevent clotting or bleeding.
- Collaborating with multidisciplinary teams, including residents, nurses, and doctors to coordinate comprehensive patient care.
Overall, this rotation prepared me on how to confidently manage a wide variety of outpatient conditions and ensure they are prepared for diverse practice environments upon graduation.

Advanced Pharmacy Practice Experience 3 (Sept. 2024)- Oncology.
Rotation Type: Oncology

Description: During my oncology rotation, I gained a deep understanding of cancer care that significantly influenced my approach to patient management and treatment. Collaborating with a multidisciplinary team, I learned about advancements in oncology, including targeted therapies and immunotherapy. I participated in patient assessments, treatment planning, and palliative care discussions, which enhanced my clinical skills. A highlight was attending tumor board meetings, where I witnessed evidence-based, collaborative decision-making tailored to individual patients. This experience reinforced the importance of communication and empathy in patient care. I also engaged in patient education, helping them navigate their diagnosis and treatment options. Overall, this rotation equipped me with essential clinical knowledge and a compassionate approach vital for my pharmacy career. This rotation came with lots of assignments, including Soap notes, researching and answering questions about certain oncology drugs and a topic discussion on iron infusion. My best work in this rotation is an order set I created for Cladribine and presented my work to the oncology nurses.

Advanced Pharmacy Rotation Experienced-Rotation# 2- Institutional Pharmacy
Rotation Type: Institutional Pharmacy

Site: Holzer Medical Center, Jackson, OH
Description: On this rotation my daily activities included attending rounds with other medical professionals (Doctors, nurses, social workers, physical and occupational therapies), placing and receiving medication orders, picking and replacing control drugs at the Emergency department, filling crash carts, attending hospital meetings, and answering questions from other medical professionals, and patients alike. My best work for this rotation was a discharge counselling I provided to a patient, and also addressing some of his concerns about Xarelto.

Advanced Pharmacy Rotation Experienced-Rotation#1- Community Pharmacy
Rotation Type: Community Pharmacy.
Preceptor: Tyler Glassburn
Site: Holzer Family Health, Jackson, OH
Description: On this rotation my daily activities included, answering calls, answering patient’s questions, counselling patients, entering prescriptions, receiving and sending transfers, pre-checked and final check of patients’ medications. It was a great experience where I learnt a lot of stuff. I was so proud of what I was able to do in this rotation. My best work from this rotation is a presentation on Pantoprazole in patients at Risk for GI bleeding in the ICU. I did a virtual 25-minute presentation with my preceptor, and one other pharmacy staff. Attached is the presentation and handout, along with rubric.
