During this rotation, there was a major backorder for Ozempic, Wegovy, and Trulicity. I had to interact with the prescribing physicians on the Epic platform to ensure we found alternative agents or routines for the patients that we could not fulfill their orders due to the shortages. |

I contacted the dentists office and spoke with the dentist. He changed the directions to "Swish 1/2 ounce every morning for 30 seconds". |

I. Description: Administering the booster Covid-19 Vaccine dose for the first time.
II. Feelings: When my preceptor, Pierre, asked me to administer a Covid-19 booster dose to a patient, I was very excited and nervous at the same time. I had to make sure the patient was eligible to receive the booster dose, process his paperwork, administer the vaccine, and follow-up to make sure he felt okay to go home afterwards.
III. Evaluation: This was a very positive experience. The booster dose was released earlier in August and I was very excited to be assessing eligibility of patients to receive it and administering it to patients who were eligible.
IV. Analysis: I felt this way because in the Spring semester, I went through the APHA Immunization Program and was excited to apply what I learned in the program to the latest vaccine dose that has been approved.
V. Conclusion/Action: I plan to stay up to the date with the guidelines released from the state on the booster dose to be best able to give patients knowledge information regarding the booster dose.
I. Description: Patient counseling on which type of laxative would be most appropriate for them. The patient has been constipated for about two days and would like to pass a bowel. They are healthy, not taking any medications, and would like something that works overnight.
II. Feelings: I was excited to be able to apply my knowledge on constipation and my knowledge on how to approach a counseling session with a patient in a community setting.
III. Evaluation: This was a positive experience because it allowed me to interact with a patient and counsel them on a very important topic.
IV. Analysis: I felt this way because constipation is a very prevalent topic in community pharmacies and as a future pharmacist, it is important to have appropriate knowledge on counseling for laxative selection and use.
V. Conclusion/Action: I plan to continue to apply my knowledge and make mental notes of I can better apply the knowledge and what pieces of information are most critical to gather from the patient.
My career goals coming into my IPPE institutional rotation was to get exposure to the workflow in an institutional setting, observe the difference in environment from a community setting, and work with different teams and departments to deliver patient-centered care & evidence-based care while collaborating with other professionals at the institution. One event which reaffirmed my goals coming into this rotation was when I attended a Teams meeting with my preceptor to discuss new patient admissions and discharges. Patient X had just been transferred to the rehabilitation center after having a valve replacement. The patient needed to be interviewed for us to obtain a current medication list. After interviewing the patient, there was a discrepancy between the hospital notes the patient was transferred from and the information obtained from the patient. In order to confirm the medication the patient was actually using for her elevated cholesterol, we needed to reach out to the mail order service the patient uses to fill her cholesterol medication. This was a great example of patient-centered care while collaborating with other professionals from the institution. Another experience which reaffirmed my previous goals was when I got the spend time with the lead technician at the institution to understand how medications are distributed throughout the rehabilitation center from the pharmacy.
Prior to starting my first rotation I envisioned myself to work in a manner which produced high quality work. I intended to follow the saying, "Quality over quantity". After starting my rotation, I realized this is one of the key characteristics to succeeding in a community pharmacy. High quality of work, patient interactions, verification, and much more are all essential to creating a successful workspace for the pharmacist and the pharmacy. Two instances which reaffirmed the importance of high quality work included: Last week when a patient dropped off a prescription for prednisone, but the directions on the prescription were not correct for the diagnosis and treatment. I contacted the doctor's office and was able to verify the correct directions for the prescription. Another instance is when we receive the controlled medication orders every morning. It is important to go through every box in the shipment to verify the correct items and manufacturers were sent to the pharmacy.
