This month I completed a hospital pharmacy rotation focused on inpatient clinical pharmacy services. I participated in daily ICU rounds and was involved in medication management including warfarin dosing and INR monitoring, vancomycin dosing and level monitoring, renal dose adjustments, and antimicrobial stewardship. I also learned more about TPN, formulary review, and hospital medication distribution systems. I completed a COPD case presentation and SOAP note as well as an SBAR presentation and class review on antiviral agents. This allowed me to apply clinical knowledge and better understand pharmacy practice within a hospital.

This month I completed a clinical pharmacy rotation in general medicine. I participated in daily patient rounds and reviewed patient profiles, laboratory values, and medication regimens. I completed weekly topic discussions, SOAP notes, and a patient case presentation, which allowed me to apply clinical knowledge to individual patient cases. I also gained experience with warfarin dose adjustments and INR monitoring.

During my oncology rotation, I participated in daily rounds with the hematology/oncology team and observed patient care and treatment planning. I completed “drug-of-the-day” projects, presented a topic discussion on chemotherapy-induced nausea and vomiting (CINV) and expanded my knowledge of oncology therapies, supportive care, and the pharmacist’s role in managing patients with cancer.

This month I completed a clinical pharmacy rotation in a level IIIB NICU. I participated in both low-risk and high-risk rounds, reviewed patient medication regimens, and calculated weight-based dosing for infants. I completed a journal club presentation and a surfactant drug class review while learning the pharmacist’s role in the NICU. Attached is the journal club presentation.
