Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: During this rotation I got to be apart of the PICU & pediatric floor rounds every morning and was able to experience how a team of nurses, pharmacists, doctors, residents all collaborate together to better patient care and outcomes. I was able to assist in pediatric dosing for antibiotics and other medications and provide recommendations in medications and dosing.

Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: During this rotation, we had a patient who was feeling the effects of methadone severely due to a concurrent therapy with an anti-fungal which was a CYP inhibitor therefore increasing the levels of methadone in the plasma. We had recommended an alternative medication to avoid this issue and provided education to the providers with a presentation on CYP enzymes and potential drug interactions.

Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: During my rotation I had went on an SNF visit at nursing home where we collaborated with a social worker, pharmacist, physician assistant, care coordinator and etc. We discussed many patients in depth that had just arrived or were about to be discharged and we worked through an initial plan and an exit plan on how to best accomadate patients at home, in the nursing home, transportation and medication/medication costs.

Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: I recently called a doctor regarding a prescription that was sent in for an 18 year old female that had strep and was prescribed penicillin vk 250mg/5ml solution but it was not covered under the patients insurance. Therefore, I called and recommended to the doctor on call to switch the medication to amoxicillin 400mg/5ml and was able to provide a dosing equivalence together.
