Briefly describe the encounter and counseling you provided for the patient:: | A patient presented to the ICU after their sodium levels decreased significantly. Prior to the ICU, the patient was experiencing nausea and vomiting, along with severe fatigue. Once admitted and transported to the ICU, lab values returned and the patient's sodium levels were 125 mEq/L, which was severely low. Their CrCl was slightly below normal limits and their Potassium levels were good. Also, we collected pass medical and medication history. After discussing and assessing with the pharmacist and the other healthcare professionals, it was recommended to start this patient on NS (Sodium Chloride) 50ml bolus every 10 minutes with the goal of increasing these levels to roughly 135-140 mEq/L. Before starting, the pharmacist and I counseled the patient on the goals of therapy by informing them about trying to increase their sodium levels by 4-6mEq/L over a 24 hour period, along with potentially experiencing side effects such as fatigue, headaches, and confusion. This was due to the low levels, but once their sodium levels would rise, these symptoms should not be present. Once counseled, the NS was started and we followed-up with the patient several times over the course of the day and continued regimen until labs were taken the following day to assess the changes in sodium. Once lab values returned, we assessed the sodium levels. |
