Today’s lecture about pharmacist’s ethics provides many aspects one needs to know and be able to perform in order to provide safe, effective healthcare services while maintaining veracity and fidelity. A pharmacist needs to practice ethical codes every day - which are beneficence, non malfeasance, justice, autonomy, veracity and fidelity - so when a crucial situation arises, he/she will be able to keep it under control with the right decisions. It’s not an easy thing to do especially when dealing with patients who are struggling with illnesses and are not fully updated to the current knowledge.
The ethical rules and principles were shown in the situation discussed, whereas the patient and her family were looking for a treatment to improve her health status and her addiction. The PCP failed to perform fidelity as he did not make an effort trying to apply for a DEA waiver to help the patient. While the pharmacist was reluctant to cooperate with the PCP, she didn’t want to attract wrong customers to the pharmacy and enable the patient’s habit, she has practiced her ethical rules which are non-malfeasance - to do no harm and being truthful - veracity. The EMTs on the other side are doing their work voluntarily, they have shown justice and autonomy - to provide care to everyone in need.
I have seen many similar experiences where the pharmacists faced challenging situations with patients demanding their medications and prescriber not being able to help much. Though some approaches are upsetting, they did their best to follow the ethical rules as well as providing the best interventions/options for patients. Being a pharmacist isn't going to be easy, and I am going to need a lot of practice.
Our group project was on the Smoking Cessation program which targets smokers in general, especially young adults in developing countries, as studies have shown a significant rising trend. The main critical factors that we considered when selecting this entrepreneurship project was to define the program, conducting market research and analyzing competitors - in order to help reduce smoking in all populations as well as its related health issues, mainly focusing on teenagers and young adults. This project has a lot of potential. I’m currently working at a retail pharmacy and the rate of patients getting nicotinic aid for smoking through my observations are increasing, and patients pick up their gums, patches or lozenges pretty frequently, which shows the understanding in helping patients quit smoking as well as improving the target goal of the initial project. I’m very happy and excited to see what will come later on in actual practice when I can counsel my patients on the effects of smoking and advising, helping them to quit. Patient outcomes will be improved, quality of life of patients will be better, they can save tons of money on quitting cigarettes/smokes. I’ve associated with lots of smokers and I’ve gotten 5 people to quit, it’s a long process but it is a good start so far. When offering this service, not everyone will listen, and quitting is hard so talking them into it is just as hard, you don’t want to aggregate them into thinking what they’re doing is wrong, being sympathetic to know where they come from, why they started smoking in the first place, and find out if they ever thought about quitting, hence they way we advise them is very important. I’m grateful for this opportunity.
The purpose of this activity was to help us with self-awareness as well as getting a sense of developing leadership behaviors and values, which in turn allow us to demonstrate the responsibility one holds in creating and achieving goals, regardless of position. This series explains what pharmacy residency is and why we should choose to do residency. I’m very interested in an opportunity as I could work with many challenging aspects of clinical practices as a clinical pharmacist. It could be an experience for me to challenge my ability to think critically in acute care settings. The speaker was very thorough in explaining the research for certain programs, application, interview process, and preparing for interviews. She also portrayed well how it's going to be as a resident. The tips for success are to stay active, to reach out to faculty on campus and residents, to apply for intern positions and to stay organized. She also mentioned to make the best out of our upcoming rotations. I'm excited for what will come. Overall, this was a very great activity and I'm looking forward to my APPE, hopefully I can find out a specific area that I'm really into.
This activity shows a series of sessions for graduate students who are or will be interested in learning about postgraduate education opportunities, how to get the most out of a mentor program with the application process and what you can do to better position yourself for these opportunities. The activity pointed out tips and tricks to communicate, manage time, use positive interpersonal communication skills, and organize with a mentor. I really appreciate this session, even though I’m not planning to go into postgrad education, I think it is great for not only those who are but also for me to know about what I can do, to further develop my interest in all aspects of pharmacy. A mentor would be able to help with my personal as well as professional development, along with communication and building my connection which I think is very important in identifying and achieving my career goal. Overall, this was a good experience.
The activity shows the overall presentation of pharmacy market in the US in the past 15 years as well as the independent pharmacies. I recently interned at Stop and Shop pharmacy. Unfortunately, that location was shortly closed down after my rotation due to not getting enough compensation for drug prescriptions from insurance companies. And I’ve wondered as it is difficult for chain pharmacies to stay open, how independent pharmacies would be able to start up and operate ownership. This activity was very helpful in helping me answer my question. One who wants to open their own pharmacy has many things to consider like physical availability, personal interests and goals, competition, and most importantly financial options which include conventional, wholesaler, personal, junior partnership, seller note or any combination of these. The average opening cost is pretty high, it also takes some time for independent pharmacies to build customer base, they probably have to defer their first couple years salaries to maintain the pharmacies. The neighboring pharmacies are very competitive as well. However, there are many advantages if you are to take over an ownership, you have the knowledge of financial history, there are many opportunities prior to ownership, you are paying yourself from the day of opening, customer base and brand recognition are already established. There are many options in buying a pharmacy, though I don’t plan to open my own pharmacy yet, it was great to learn about all options I can encounter after graduation.

Write Reflection Here:: | My community rotation site is Stop and Shop Pharmacy. I've worked in retails so I know what to expect. However, I've got to learn a lot more here than I've ever would at my local store due to a busy work flow. It's been a great two weeks. The pharmacists there are all very helpful and enthusiastic with their work. Not only did they show me what to expect as a future pharmacist, they put me on task so that I can learn better. Besides the basic every operations, the pharmacists let me do other things. I've been making calls to insurance companies, patients, I've communicated with actual hospital team members about a patient related situation. I was also able to give out vaccines to those in need. Everything I was taught over the last two weeks confirm how much I can handle on my own. It's a great experience so far. |

Write Reflection Here:: | I've always been working at a retail/community setting pharmacy therefore interning at an institutional/hospital setting was really an eye-opening opportunity for me as well as my colleagues. During the last four weeks, I've got to learn a lot of new things which to me is very important in deciding whether or not I want to work at retail or clinical pharmacy. Though I was kind of hoping to interact more with patients, get to experience on taking their medical histories, explaining or counseling patient about their medications, I didn't have the chance to do so rather than working with automated prescription dispensing machines or omnicell - which I really enjoy working with. Nevertheless, this formal training to become a license pharmacist taught me well. I also appreciate learning about the hospital medications, which some of them are not usually found at a community settings. The rotation was short and it went by fast, I hope to have a chance to work with my interns and preceptors again in the future. I'm grateful to be able to work for the pharmacy and brings what I have to offer help and support to the pharmacy. |

Write Reflection Here:: | I've been working at my first institutional IPPE rotation site which is Spaulding Rehabilitation Hospital for two weeks now and so far, I'm loving it. Everyday I get to learn new things and get involved in an actual work schedule of an employee. I've always been working in a community setting, therefore to be able to work in a hospital with inpatient setting, it is a great learning opportunity. There are always normal restocks, demand orders, specified orders, in which I feel most engaged when I'm out delivering medications to each specific unit during the day. I get to communicate with the patients as well as nurses and other employees who work there. Though patients are struggling with medical problems, nurses are running back and forth and doing their most to help patients, they're always smiling and in a good mood. Not only does that help me developing my communicating skills, it shows me patience and empathy and responsibilities each must own when doing their tasks. I feel excited coming to work everyday. It is definitely a positive experience. Along with being able to work on the floor individually, but working as a team bring about many great impacts and I truly appreciate it. I feel confident in everything I do, but not too confident that I wanna be able to learn stuff from other people, especially the resident students, technicians and pharmacists. Everyone who work at SRH and those who are getting treated create a great environment where patient outcomes are centered. I will continue to work on my practices, open to learn so one day I can be able to offer my work more than just what I could do now. |

Write Reflection Here:: | I am truly thankful for this opportunity at Stop and Shop. This 4-week rotation taught me so much of things I need to know. I was able to do certain tasks on my own, got to deal with customers and counseled them on how they should be taking their meds, followed up with them to make sure they're getting the most benefits out of those medications. We're an expert as drugs and I was glad patients come to us and ask for they can take over the counter before going to the doctors and I feel great knowing I have quite a good amount of knowledge to show them what they can take. It was great because I got to work with many floaters who work differently and have different perspective about many aspects, but overall the goal is to pursue better patient health outcomes. Everyone was so nice overhear, making this rotation a memorable experience. This really proves how much I love working as a pharmacist and I really hope to come back here and become an official staff one day. |
