Description
This month I worked at the Bing oncology infusion clinic and the inpatient oncology unit.
During my time at the infusion center, I completed patient workups. I assessed patient treatment plans using NCCN templates, performed dosage calculations including carboplatin dosing, recommended dose reductions based on performance status and renal function, and evaluated non-chemotherapy infusions. I spent some time shadowing nursing and sterile compounding. I also performed literature evaluations when treatment plans did not match the nccn template
My best work this month was my case study presentation.

Description:
This month I worked in an offsite location of Nationwide Children's Hospital located inside of Riverside Methodist Hospital. Nationwide is responsible for staffing all NICUs within Columbus Ohio and several other locations. I worked closely with Dr. Martinez and rounded with the interdisciplinary team. This month was complicated by the fluid shortage due to Hurricane Helene. This allowed me to use clinical decision-making to ensure appropriate populations received TPNs and other fluids. I attended bi-weekly lectures provided by residents and pharmacists. I participated in antibiotic dosing and de-escalation, caffeine dosing, fluid and electrolyte management, immunization schedules, and neonatal abstinence syndrome management. My best work this month was my Journal Club on Patent Ductus Arteriosus.

Description: During the month of September, I was with Dr. Barga in the Hepatology clinic. This was a unique experience where I had the pleasure of leading Hepatitis C treatment visits. I was able to participate in topic discussions including HBV, HCV, cirrhosis, hepatocellular carcinoma, primary biliary cholangitis, and a journal club on the RESPONSE trial. I also had the pleasure of attending an interdisciplinary tumor board. My best work this month was my journal club.

Description: For August, I was with Dr. Brianna Thomas on one of the internal medicine floors, 3 orange. On this floor, I was able to perform patient workups, attend multidisciplinary rounds, and make medication recommendations as necessary. I performed various renal and kinetics dose adjustments such as Vancomycin, Warfarin, and Lovenox. I was able to ensure transitions of care through AVS summary review, iVents, and progress notes. My best work for this month was my Drug Information Question response which was a preliminary review for a change in Apixiban dosing in HD patients for Ohio Health.

Description:
For this rotation, my time was divided between Riverside Medical Building Outpatient Pharmacy (RMB) and RxCare Charitable Pharmacy. During my time at RMB, I was able to help with order entry, automated dispensing, verification, and meds-to-beds clinical counseling. This helped me gain clinical knowledge for counseling during transitions of care. During my time at RxCare, I was able to help enroll patients in manufacturer assistance programs, educate medical residents and nursing staff on inhaler technique, observe dermatologic clinic visits, and recommend treatment for patients presenting to their PCP. My best work this month was my presentation on probiotic supplements including medication coverage and availability in the outpatient setting. I created a PowerPoint presentation as well as a handout with treatment guidelines from WGO.

For my Spring 2022 IPPE experience, I chose to complete my hours with Kimberly McGinnis at Genoa pharmacy in Toledo, Ohio. This was a unique community pharmacy experience because all Genoa Pharmacies are located within mental healthcare facilities. This particular location is located within Unison Health. This experience was insightful because it allowed me to envision the entire healthcare process. With the prescribers sharing the same hallway as the pharmacy, we were able to see the patients directly after the prescribers and we were able to walk a few feet to consult the prescribers as well. I was able to perform various workflow functions as well as other atypical functions such as working with insurance and consulting patients on taking their medications properly.

For my Fall 2021 IPPE experience, I completed my hours with Melissa Sherman at Meijer Pharmacy in Bowling Green, Ohio. Since this is a larger chain pharmacy, the volume of business was a bit higher than it would be at an independent pharmacy. At this site, I spent a good amount of my time counting and packaging medications, answering phone calls, typing prescriptions, and scanning documents into the computer. I became very familiar with work-flow within Meijer pharmacies and how their systems function.
This course is designed to introduce students to pharmacy topics that are related to professionalism and prepare students for IPPE experiences. This course has provided me with many experiences involving cultural competency, safety, adherence, law, and other various topics. This class has helped me gain the necessary skills to perform better during IPPE experiences.
