Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: A great professional interaction I had this past week was with Traci Tinker watching how she interacts with her diabetic patients. She was counseling the patient on the importance of taking their metformin twice a day to lower their blood sugar and was very thorough and even asked for my help in properly dosing the medication and any adverse effects that could happen with the medication.

Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: During my first set of rounds I got to interact with the ICU physician, the nursing staff, dietician, and respiratory therapist to help assess patients and their medications they were on. The intervention I got to conduct was based on a patient's antibiotic therapy and their CrCl. The patient had cellulitis and was put on cefepime and vancomycin to help the infection. The cefepime was dosed at 2g every 8 hours. The patient's CrCl was 49.3 mL/min. After speaking with my pharmacist preceptor (Joe), we concluded that we needed to increase the dosing interval from 8 hours to 12 hours to help save the patient's kidneys. I expressed this to the physician with the help of my pharmacist and the physician enjoyed our input and changed the medication to fit the patient's CrCl.

Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: My IPE experience involved me rounding on the second floor med/surg floor with a NP and physician within Falmouth Hospital. I sat down with both of them and reviewed all patients on the floor, reviewed their current symptoms and their medications and discussed add on therapies and what patients need to have medication changes. Most orders were appropriate but I did add recommendations when needed and they were double checked by the NP calling down to the pharmacists to ensure everything was checked out. We interacted with all patients which gave me great exposure to how patients take their meds and how they look and feel while being in the hospital. One specific patient was an interesting case. The patient's family wanted to put her on morphine after a seizure. The patient was 70 years old and had a very poor quality of life according to her daughters. The daughters were her primary care takers and her healthcare proxy. They wanted to put their mother on a morphine drip and ensured us this was their mother's wishes if she ever got to this stage. The NP and MD wanted to wait for the Hospice nurse to come in before approving anything. The Hospice nurse came in and discussed the situation with the NP, MD, myself and the family. The patient was ultimately put on morphine for care and comfort and I looked over the order to ensure it was appropriate for the patient. I did not make the order, but rather looked it over and made sure the dose matched the indication and use in UpToDate. After talking over the situation with the NP we continued to round and talk with patients. Although I was not able to assess patients because of my scope of practice, it was nice looking and understanding the different conditions each patient was dealing with, as well as how the NP assesses each patient.

SPPD III Leadership reflection (P2: Summer)

1. I feel I successfully incorporated the philosophies of leadership through autonomy and leadership through teamwork instead of a hieracrchial system. I feel as though these are two of my strong points when it comes to leadership because I realize that not everyone can do something exactly the way I want it done. Giving my classmates the freedom to look up information and incorporate it into the document made the entire exercise much easier. Because of this, the activity went smooth and we were able to get mostly everything correcy from the case in half the alotted time. Furthermore, incorporating teamwork and emersing myself into the team instead of acting like a supreme leader allowed us to be more efficient. I feel as a leader, it is better to do as you say. I do not like the idea of barking out orders then sitting back and waiting for your team to drop the information in front of you for evaluation. Instead, giving everyone a role and allowing them to fulfill that role and work together to help each other complete our tasks served to be more beneficial and made the activity more enjoyable.

2. Two elements of leadership that I need to work on are those of communication and organization. Instead of assigning specific roles to people, I sort of let everyone incorporate their own ideas into the word document. However, this led to some confusion as some people were completing the same tasks as other and it turned out to be less effecient then we anticipated. Secondly, as a communicator I needed to be more vocal in making the final decision as to what to do. I am not saying I wanted to dictate every decision being made, but when we had to two teammates professionally arguing over what the diagnosis and treatment guidelines were, I needed to step in more and not only help come to a conclusion, but also settle and re-focus the group to help accomplish our goalsl.

Write Reflection Here:: Two experiences that reaffirmed my goal to become a clinical pharmacist or an MSL that can discuss certain aspects of clinical research were both P&T Committees we sat in on. These meetings were informative and showed me how important a clinical pharmacist's role is in helping patients in institutions. I was able to listen in on the conversation of how formularies are made and the data behind them that reconfirms certain medications on the list and explains why others are left off, either for monetary or safety concerns associated with the substance. It was also really interesting to see how all the data was collected and analyzed. Numbers were crunched on a monthly basis to assess the money spent on certain medications that were deemed highly important, like HIV and Hepatitis C medications. These numbers were used to set budgets and help show the need for certain drugs for certain patients and how the healthcare team can work cohesively to accomplish the goals of getting the right drug to the right patient for an affordable cost. Overall, both experiences were helpful and informed me on the importance not only the pharmacist plays in assessing clinical situations and costs, but how they can work in a healthcare team to better help the entire patient population throughout multiple hospitals and institutions.

Please provide one example of an interprofessional observation and/or interaction that you had during this IPPE rotation in one to two sentences.: We sat in on a few different P and T committees where we got to understand how drugs are added and removed from formularies. Not only that, the committee resolved problems involving adverse events and medications not being sent out in time to desired sites. They also looked over all the medications that were sent out for the month and how to improve medication roll out and stop errors from occuring before medications are sent out.
